You get a glimpse of what she actually looks like when she releases the chakra from the diamond seal on her forehead... it's pretty rough.
Suzuka: @fishmonger: Um, her energy was drained to the full scale of death because she used too much chakra to protect the village. So of course she's look rough. Even when she's fighting Madara and lost, she still looked sexy.

But really, your argument doesn't make a whole lot of sense, because no one else noticeably ages when they use a lot of chakra. Since Tsunade constantly has a transformation jutsu activated, without chakra she wouldn't be able to maintain it and thus you would see her actual body. She doesn't look rough because of the lack of chakra or the rigours of exertion, she looks that rough because she has aged poorly.
