I think I know what is going on here.
You see, there is an (obvious) size difference between the two and as such, the ass, or (if applicable) vagina is much too big. The only other available hole in the urethra(Holy crap I spelled the right), and as such, will be used. I suppose it depends on someone's personal kinks, however, this is not uncommon as some would like to think.(That being, the idea of the urethra being used as a substitute when there is no vagina available to fuck on said person.)
Now, I personally don't mind this as I am used to it, and, quite frankly, think it is a bit arousing myself.(Not this picture, but the a fore mentioned idea.)
