My first thought is that he manipulated the actual figure from the game (like a lot of people do with 'SFM Porn', I think its called).
However, I swear I recognize the breast morph used (my primary interest ^_^), particularly the nipples, which means he combined a Daz Studio render with an image from the game, with some very good Photoshop postwork.
FUCKHEAD: @Chupacabra555: It's both, actually. I import the head from the game into DAZ Studio, where I combine it with the body and then edit the textures and shaders to fit.
