All i know is, that people hacked the game and were able to remove the underwear, and from what i've read here and there, those nipples are real Namco textures.
...and looking at what revealing costumes and armor the models in SC5 can wear wear, i see absolutely no trace of the grey triangles they used to censor the nipples in SC4, when in SC4 if you'd wear certain revealing tops, you could kinda see the edges of those grey censors.
So i think this might actually be true.
