E.g., "to get dirt in one's eye" means that dirt is irritating the eye's surface, it does not mean that the dirt is actually inside of the eyeball or behind it (i.e., in the socket), and similarly if you spit "in someone's eye," it doesn't mean the spit will make it into the socket or actually enter the eyeball.
