Anonymous2: Well, all the characters(barring the Kappas) drawn by... Whoever the artist who makes these Kappa Girl pictures is, all seem to be based off Japanese Mythology.
As such, I'd assume the guy with the second mouth is based somewhat on the "Futakuchi-Onna", a woman-turned-Yokai with a second mouth in the back of her head. The prototypical Futakuchi-Onna was the wife of a Miser who never ate, so she became cursed with a second mouth in her head that ate twice as much as what the original mouth eats... Not to mention the second mouth can apparently make the woman's hair become prehensile to feed itself.
Of course, I've seen other picture involving that Futakuchi-Onna-based character, and it's a guy... So it'd probably be more like Futakuchi-Otoko.
KDKC: @ Loki: They're calling her bald ("Hage"), due to the spot on her head without any hair where said water would rest. The Kappa is yelling that she's not bald. (It's normal for her species, after all)
