Ok, I might be over thinking this sometimes, but I have played some really amazing mods for FO3 and FO:NV with whole adventures and animations and graphics and sex and BDSM and so on, so I imagine it can be done. But what I'd love to see is a mod along this fantastic picture, a mod about say... Vault 181 (it's metric for 69 - think about it). Vault 181 has three classes for its particular experiment on domination and slavery.
Class 1: Executive class - Lead by the Overseer, these are the heads of the various departments within the vault and are the dominants/masters of the vault with replacements identified via a specialised G.O.A.T and trained as such (including a special course of being subjugated by your superiors only for a given period so they know how certain toys, clamps, floggers, etc feel - some may even convert to liking it!). These people will command the 2nd class and tease, torture, tantalise, and use any and all of the 3rd class as is their whim.
Class 2: Technical class - Security, technicians, doctors, scientists, etc. These assist the executive class members to undertake in their particular pleasures and like the 1st class minority, are trained after the G.O.A.T has determined their suitability as being. They have access to using the 3rd class just as the executives can, but usually numbered to a given pool of them. They of course can and have been punished for small crimes and and breaking of rules as if they were 3rd class dwellers but usually on a limited basis. A more serious crime of course can see them forever lowered to the 3rd class rank which is nearly impossible (but not unheard of) to get out.
Class 3: The slaves and toys - The more attractive and desirable populace are used as toys by the executive class, the others are the drudges and labour force of the vault. They are the kitchen hands, the cleaners, maids, servants, and anything that does require technical skills and education. They are no longer educated from the time they failed to achieve anything useful in their G.O.A.T. They are kept mostly naked and when not working, they are either bound tightly or locked up in some less than ideal fashion, often in a most uncomfortable way.
Toys are used at will by the executive while the slaves are used in their down time by the tech class. Their food is bland but with enough calories and protein so as not to cause illness or weakness (not to make them useless as worker drones!). They are subject to the tortures and more as depicted in this graphic and undergo psychological and conditional training to accept for the most part what is done to them - but they don't have to like it one little bit, they just don't know how to revolt as slave collars of all kinds are employed to make it futile (exploding, shock, magnetised that pulls them against the wall or even a floor if they stumble, tranq, and so on).
This is a mod I'd like to see where the PC can enter and either become part of the executive and do this sort of thing (with quests attached before moving on but always welcomed back as a new base of operations or home) or to be captured and become a sex slave/pet (Skyrim even has one where YOU can be the slave, used and abused by your companions) undergoing certain quests to earn your eventual freedom and still being able to return to be a voluntary 'pet' when the mood takes you (you kinky thing you). But I'll settle for sits like this if it never happens.
